Brisbane South PHN works in partnership with health professionals and our communities to improve the health outcomes of everyone in our region.

Brisbane South PHN (Primary Health Network) works in partnership with health professionals, service providers and local communities to support the best possible health and wellbeing outcomes for every person in our region.

As one of 31 Primary Health Networks (PHNs) established nationally by the Australian Government, we put our region's people and communities at the heart of health care to ensure they receive the right care, in the right place, at the right time.

We do this by identifying health gaps and commissioning programs and services to respond to the needs of our region – particularly for populations at risk of poorer health outcomes. We also support primary health professionals with ongoing education and professional development.
